Can You Use Old Vaccine Records for Form I-693?

Old vaccine records can help you avoid repeat vaccines, but only if the office can use them.

That means the record has to be more than a memory or a short list of vaccine names with no dates.

What makes an old record usable

CDC says vaccine documentation should come from a vaccine record or medical chart and include the dates the doses were given. The record also needs to look consistent and reliable.

  • Specific vaccine names or recognizable abbreviations
  • Month, day, and year when available
  • A readable format
  • No obvious signs of alteration

What an old record cannot do

An old vaccine record does not replace the rest of the immigration medical exam. You may still need current lab testing or an in-person exam even if your vaccine history is complete.

Bring every version you have

If you have partial records from schools, pharmacies, prior doctors, or overseas booklets, bring all of them. Several incomplete records are often more useful than one perfect-looking summary with missing detail.
