Immigration medical exam

What the immigration medical exam is and what to expect.

Most people filing Form I-485 need Form I-693 completed by a USCIS-designated civil surgeon. Here are the basics before you book.

Who usually needs it

Most adjustment-of-status applicants need Form I-693 completed by a USCIS-designated civil surgeon.

What happens at the visit

The civil surgeon reviews your records, examines you, and may order lab tests or vaccines based on your age and history.

Why timing matters

USCIS generally requires Form I-693 with Form I-485 as of December 2, 2024, so it helps to plan the exam around your filing date.

What to bring

  • Government-issued photo ID
  • Any vaccine records you can find, even older or foreign records
  • TB paperwork if you have had a positive TB blood test before
  • Payment information and insurance card if the office asks for it

One current rule to confirm

If the civil surgeon signs your form on or after July 3, 2025, USCIS accepts only the 01/20/25 edition of Form I-693. Ask the office to confirm they are using the current edition before your appointment.
